First Visit

Consultation

smiling older man at dental officeYour initial appointment will consist of a consultation with Dr. Henderson.  During this appointment the reason for your visit will be reviewed, and include a head and neck exam with an explanation of your diagnosis and the various treatment options available.  Occasionally, the procedure can be performed on the same day as the consultation. However in certain instances, such as a patient with a complex medical history or treatment plan will instead require an evaluation one day and a second appointment to provide treatment.

We provide two methods of patient registration.  The first of these can be found in the Online Forms section of this website, while the other is to come in to our office and fill out the necessary forms.  Whichever method you choose, please assist us by providing the following information at the time of your consultation:

  • Your surgical referral slip and any x-rays if applicable.
  • A list of medications you are presently taking.
  • If you have medical or dental insurance, bring the necessary information. This will save time and allow us to help you process any claims.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or court appointed legal guardian at the consultation visit.

A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. Please have nothing to eat or drink at least six (6) hours prior to your surgery. You will also need an adult, with a valid driver’s license to accompany you to the office and drive you home.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-rays

If your dentist or physician has taken x-rays, you may either bring them to your appointment or request that your doctor forward them to our office. If there is not enough time for the films to arrive at our office before your appointment, please pick them up and bring them to our office. If additional films are necessary, they will be taken at our facility.
